  1. Field trip form ( to get on bus both ways)
  2. Registration form ( to get off bus and go to camp)
  3. Medical  forms - as needed, to include physician's authorization for any prescription or over the counter medication to be given to student.  If aspirin were to be sent in case of  headache, there needs to be an accompanying authorization from a physician.
  4. Get forms
